Telstar 5

Proton-K/DM-2MBaikonur Cosmodrome, Republic of Kazakhstan
Drawing of the Proton-K/DM-2M for Telstar 5

Carried the Telstar 5 communications satellite, but a failure of the Block DM-3 upper stage left it stranded in a useless high-inclination orbit; the insurance write-off was later acquired by Hughes and repositioned via lunar swing-bys to become the HGS-1 satellite by mid-1998.
