Ariane 42P failed carrying PanAmSat's PAS-3 satellite from Kourou, with the third-stage (H10) gas generator malfunctioning about 15 minutes after liftoff and destroying the spacecraft — the first failure of the Ariane 42P variant. PanAmSat claimed insurance and later launched a rebuilt spare, PAS-4.
