Titan II SLV launched USA-45 (SBWASS R2), a US signals-intelligence satellite, from Vandenberg's SLC-4W. The satellite's boost motor failed to fire, leaving it stranded in its roughly 200 km parking orbit, and it reentered shortly after launch.
