Vanguard TV-5, a Naval Research Laboratory test flight with a ~10 kg X-ray/geodetic satellite, launched from Cape Canaveral LC-18A on 29 April 1958. The first two stages performed, but the solid third stage failed to ignite after separation and the payload did not reach orbit.
