Cygnus CRS Orb-3 (S.S. Deke Slayton)

Antares 130Wallops Flight Facility, Virginia, USA
Drawing of the Antares 130 for Cygnus CRS Orb-3 (S.S. Deke Slayton)

Antares 130 exploded about 15 seconds after liftoff from Wallops Island when a turbopump failure in an AJ-26 first-stage engine triggered an in-flight fire; range safety destroyed the vehicle, losing Orbital's Cygnus Orb-3 cargo spacecraft and damaging the launch pad.
