Astra's Rocket 3.3 (LV0008) suffered a payload fairing failure shortly after liftoff from Cape Canaveral on the ELaNa 41 mission; the fairing failed to separate, the second stage ignited while still enclosed and tumbled, and the flight was terminated, destroying four NASA-sponsored CubeSats.
