PSLV-D1, the first flight of India's Polar Satellite Launch Vehicle, carried the IRS-1E remote-sensing satellite. A software error in the guidance computer's pitch control loop caused loss of attitude control, and the rocket crashed into the Bay of Bengal about 12 minutes after liftoff.
