Phobos 1, one of twin Soviet probes launched from Baikonur on a Proton-K/D-2 bound for Mars and its moon Phobos, was lost on September 2, 1988 after a ground-uploaded software error deactivated its attitude thrusters, causing loss of solar-array pointing and battery depletion.
