OPS 2921 (KH-4A Corona mission 1005), a U.S. NRO film-return photoreconnaissance satellite. Thrust-Augmented Thor Agena-D from Vandenberg lofted it into LEO on 27 April 1964, but film breakage, Agena power loss, and a failed SRV separation left the capsule to reenter over Venezuela with unusable imagery.
