Proton-M Briz-MBaikonur Cosmodrome, Republic of Kazakhstan
Drawing of the Proton-M Briz-M for Arabsat 4A

Carried the Arabsat-4A communications satellite, but the Briz-M upper stage shut down about 27 minutes into a planned 31-minute burn after a valve malfunction, stranding the satellite in a useless orbit from which it soon decayed. It was Proton-M's first launch failure.
